Prime Physical Therapy & Sports Care LLC assists patients' return to their physical health and increase their quality of life by treating and educating them with state-of-the-art, comprehensive and compassionate physical therapy services.  We emphasize open patient communication and individualized treatment programs that help patients recover from injury or surgery, promote wellness, and return to an active lifestyle.

Dr. Jonathon Corbacio PT, DPT
Doctor of Physical Therapy / Owner

Jonathon graduated from Misericordia University with his Masters of Science in Physical Therapy in 2011 and began his career working in a private outpatient physical therapy practice in Monmouth County. While working full time, he went on to earn his doctorate in 2014. Jonathon’s doctoral work and research focused on the prevention of non-contact ACL injuries in the female athlete. Jonathon has spent his career refining his skills through independent research, continuing education, and certifications focused on Sports Medicine rehabilitation. Some of these certifications include FMS-1, STMT-1, and speed/agility/quickness training. While Jonathon is passionate about sports rehabilitation, he is also extensively experienced in working with all patient populations including various orthopedic, neurological, and cardiopulmonary conditions. Jonathon takes great pride in developing a personalized physical therapy plan of care that caters to each of his patient’s individual needs and lifestyle.

 

Prime Physical Therapy & Sports Care has been a dream of Jonathon’s since the beginning of his career as a physical therapist. With a background in track & field, football, baseball and basketball, he is able to apply his knowledge and personal experiences to his patients and their respective sports. Jonathon’s vision for Prime Physical Therapy & Sports Care is to provide excellent one-on-one individualized treatment from a Doctor of Physical Therapy in a state-of-the-art physical therapy clinic.

 

In his free time Jonathon enjoys the beach, watching sports, snowboarding, playing drums, DJ’ing, and producing dance music.

Dr. Cody Bello PT, DPT
Doctor of Physical Therapy

Cody has been a physical therapist for six years and joined Prime Physical Therapy & Sports Care in 2020. He grew up locally in Howell, New Jersey and attended Colts Neck high school where he played football. His interest in physical therapy started while growing up and playing multiple sports and witnessing his teammates deal with serious sports injuries. Cody received his bachelors in Exercise Science degree in 2014 from Rutgers University.  And then went on to receive his Doctorate of Physical Therapy from Mercy College in 2018. Cody performed his capstone research on ACL injury prevention screening for female athletes using 2D video analysis. Most of Cody’s post graduate work and continuing education has focused on the proper treatment and safe return of athletes to their designated sports. Cody is currently certified in BFR, TPI-1, KT taping, cupping and IASTM. Recently, Cody received his certification from the Titleist Performance Institute (TPI) to enhance his knowledge in rehabbing patients back to golf. Cody is married with two children and resides in Manasquan, New Jersey. In his free time, he enjoys watching Rutgers and Giants football, and taking his children to the beach.

Dr. Rachel Wolff PT, DPT
Doctor of Physical Therapy

Rachel graduated from Rutgers in May 2022 with her Doctorate of Physical Therapy degree. She grew up playing travel soccer and dancing competitively locally in Marlboro, NJ. Rachel’s interest in physical therapy began during her senior year of high school when she began lifting weights and joined a CrossFit gym. She became interested in movement and using exercise as a means to rehab individuals from their injury. As a PT student, Rachel trained for six months under a PT that specializes in ACL injuries. This exposure led Rachel to develop a deeper interest in treating ACL and other sports injuries across all age levels. Rachel has taken the Selective Functional Movement Assessment (SFMA) Level 1 course, which enhanced her knowledge of movement patterns and enables her to use functional training to return patients back to doing what they love. In her free time, Rachel enjoys hiking, visiting national parks, lifting weights, running and watching her favorite football team, the Philadelphia Eagles.
